Why this applies to Demeclocycline

Demeclocycline falls under P-GLYCOPROTEIN SUBSTRATES — the group Milk Thistle interacts with.

Theoretically, milk thistle might increase the absorption of P-glycoprotein substrates. However, this effect does not seem to be clinically significant.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.

Here's the gist with milk thistle and demeclocycline. This antibiotic is moved around the body partly by a transporter called P-glycoprotein. In a test tube, milk thistle can block that transporter a little, which in theory could let slightly more demeclocycline get absorbed. More drug on board could mean a bump in the usual side effects you'd see with a tetracycline antibiotic, like nausea, an upset stomach, or more sun sensitivity.

That said, I wouldn't lose sleep over this one. The evidence is mostly lab-based and theoretical, and a human study with a similar drug (digoxin) showed no real effect. It's rated minor and unlikely. Still, it never hurts to mention both to your pharmacist so they can give you the all-clear.

About Demeclocycline

What Is Demeclocycline Used For?

Brand name: Declomycin

Demeclocycline Hydrochloride tablets are used to treat bacterial infections caused by susceptible organisms. These include Rocky Mountain spotted fever, typhus, Q fever, tick fevers, respiratory tract infections (including those caused by Mycoplasma pneumoniae), cholera, plague, tularemia, brucellosis, relapsing fever, chancroid, and sexually transmitted infections such as lymphogranuloma venereum, nongonococcal urethritis, and gonorrhea (when penicillin cannot be used).

It is also used for certain skin and skin structure infections, bartonellosis, granuloma inguinale, actinomycosis, listeriosis, anthrax (when penicillin is contraindicated), and as an adjunct in acute intestinal amebiasis. In severe acne, it may be used as part of combination therapy. It should only be used for infections that are proven or strongly suspected to be caused by bacteria that respond to it.
