Beyaz is a birth control pill that pairs estrogen (ethinyl estradiol) and a progestin (drospirenone) with a folate (levomefolate). The concern on file is that milk thistle, through its active part silymarin, might gently compete with estrogen at certain estrogen receptors. In theory, that could slightly blunt how the estrogen portion acts in your body.

Here is the honest part: this comes only from lab and animal studies, not from people, and it is rated as a minor, possible interaction. There is no good evidence it weakens your pregnancy protection or causes real problems. Still, since this pill matters for both contraception and your cycle, it is smart to mention milk thistle to your pharmacist or doctor before starting it so they can keep an eye on things.

P-GLYCOPROTEIN SUBSTRATES

Theoretically, milk thistle might increase the absorption of P-glycoprotein substrates. However, this effect does not seem to be clinically significant.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.

Good news here: the concern about combining milk thistle with your birth control pill (Beyaz) is mostly theoretical. In the lab, milk thistle can block a transporter called P-glycoprotein, which helps move certain drugs in and out of the body. On paper, that could nudge the absorption of a pill like Beyaz upward a little. In real-world studies, though, this effect hasn't shown up as anything meaningful, and these pills aren't prodrugs, so milk thistle isn't expected to weaken or strengthen them in a way you'd notice.

At most, you might theoretically see slightly more of the typical pill side effects, like nausea or breast tenderness, but this is unlikely. I wouldn't lose sleep over it. Still, it's always smart to mention any supplement to your pharmacist or doctor, especially since you want reliable contraception.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Beyaz contains an estrogen (ethinyl estradiol) and a progestin (drospirenone), both of which your body partly clears using a liver enzyme called CYP3A4. The thinking is that if milk thistle slowed that enzyme down, slightly more hormone could hang around, which could mean a little more nausea, breast tenderness, or spotting. The good news: this is very theoretical. Real human studies of milk thistle didn't meaningfully change how the body handles other CYP3A4 medicines, so the lab worry mostly hasn't panned out.

I wouldn't lose sleep over this combination, but it never hurts to mention any new supplement to your pharmacist or doctor, especially since these aren't prodrugs and your birth control protection isn't expected to drop.
