Here's the gist with milk thistle and your Prefest (estradiol plus norgestimate). The concern is purely theoretical and comes from lab and animal work, not from real patients. In those studies, silymarin (milk thistle's active part) was able to attach weakly to estrogen receptors. The worry is that it could compete a bit with the estrogen in your therapy and possibly nudge it to work slightly less effectively.

In plain terms, this is rated minor and rests on the weakest kind of evidence, so I wouldn't lose sleep over it. There's no good human data showing it actually lowers how well your hormone therapy controls things like hot flashes. Still, before adding milk thistle, it's worth a quick chat with your pharmacist or doctor so they have the full picture.

P-GLYCOPROTEIN SUBSTRATES

Estradiol, Norgestimate also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, milk thistle might increase the absorption of P-glycoprotein substrates. However, this effect does not seem to be clinically significant.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.

Good news here: this is a very low-key interaction. Your Ortho-Prefest (estradiol plus norgestimate) is partly moved around the body by a transporter called P-glycoprotein, and in lab dishes milk thistle can slow that transporter down. In theory, that could let a little more of the hormone get absorbed, which might mean slightly more of the usual estrogen-type side effects like breast tenderness, bloating, nausea, or spotting.

In real life, though, this barely registers. Human studies haven't shown milk thistle meaningfully changing how similar drugs are absorbed, so most people take these together without trouble. It's still worth a quick mention to your pharmacist or doctor, especially if you notice new side effects, but you don't need to be alarmed.
