Here's the gist for Bijuva (estradiol and progesterone). The only concern on file is a theoretical one. In a lab and animal setting, the silymarin in milk thistle seemed to latch onto estrogen receptors (specifically estrogen receptor beta). The idea is that milk thistle could compete a little with the estradiol part of your therapy, which might slightly blunt how well it works.

I want to be honest about how thin this evidence is. It comes from test tube and animal studies, not from people, and it's rated as a minor, possible interaction. There's no good reason to expect a real-world change in things like hot flashes or your hormone benefit. Still, since you're on hormone therapy, it's worth a quick mention to your pharmacist or doctor before adding milk thistle.

P-GLYCOPROTEIN SUBSTRATES

Theoretically, milk thistle might increase the absorption of P-glycoprotein substrates. However, this effect does not seem to be clinically significant.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.

If you're taking Bijuva (estradiol and progesterone), here's the practical takeaway about milk thistle. Both of these hormones can be moved through your gut by a transporter called P-glycoprotein. In a test tube, milk thistle can slow that transporter down, and in theory that could let a little more of the hormone get absorbed. The concern would be mildly stronger hormone effects, like breast tenderness, bloating, nausea, or spotting.

The good news is that this is very much a theoretical worry. The real-world evidence is weak, and a human study using another P-glycoprotein drug showed no meaningful change. So most people won't notice anything. Still, since hormone therapy is personal, it's worth a quick mention to your pharmacist or doctor before adding milk thistle.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the short version on combining milk thistle with Bijuva (your estradiol and progesterone combo). Both of these hormones are partly cleared by a liver enzyme called CYP3A4. The theory is that milk thistle might slow that enzyme down a little, which could let hormone levels drift a bit higher. In plain terms, that might mean more of the usual hormone-related effects, like breast tenderness, bloating, nausea, headaches, or spotting.

The good news is that the real-world evidence here is thin and conflicting. Careful human studies showed milk thistle didn't meaningfully change how the body handled similar CYP3A4 drugs, so this is rated minor and unlikely. Still, if you notice new symptoms, or you just want peace of mind, it's worth a quick chat with your pharmacist or doctor before adding milk thistle.
