Gemifloxacin falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Milk Thistle interacts with.

Milk thistle may inhibit one form of OATP, OATP-B1, which could reduce the bioavailability and clinical effects of OATP-B1 substrates.
In vitro research shows that milk thistle inhibits OATP-B1. Two case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are OATP substrates, including sorafenib and methotrexate. OATPs are expressed in the small intestine and liver and are responsible for the uptake of drugs and other compounds into the body. Inhibition of OATP may reduce the bioavailability of oral drugs that are substrates of OATP.

Here's the basic idea with gemifloxacin (Factive) and milk thistle. Your gut uses little transporters called OATPs to help pull certain drugs into your bloodstream. Milk thistle (specifically its silymarin compounds) may block one of these, OATP-B1, in a test-tube setting. In theory, that could mean a bit less gemifloxacin actually gets absorbed, which might make your antibiotic slightly less effective at clearing your pneumonia.

I want to be honest about how thin this evidence is. It comes from lab studies and a couple of case reports involving very different cancer drugs, not gemifloxacin itself, so this is mostly theoretical and rated minor. Gemifloxacin isn't a prodrug, so there's no activation flip here. If you'd like to use milk thistle, just check with your pharmacist or doctor first.

What Is Gemifloxacin Used For?

  • Gemifloxacin is used to treat pneumonia. Gemifloxacin may also be used to treat bronchitis but should not be used for this condition if there are other treatment options. Gemifloxacin is in a class of antibiotics called fluoroquinolones. It works by killing bacteria that cause infections.

  • Antibiotics such as gemifloxacin do not work for colds, flu, or other viral infections. Using antibiotics when they are not needed increases your risk of getting an infection later that resists antibiotic treatment.
