Lesinurad falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the short version on combining milk thistle with lesinurad (Zurampic). Lesinurad is cleared from your body mainly by a liver enzyme called CYP2C9. In theory, if milk thistle slowed that enzyme down, lesinurad could build up a bit, which might add to its known risks, mostly kidney-related issues like changes in kidney function or higher creatinine.

The good news is that the evidence here is pretty shaky. Lab studies hint that milk thistle might block CYP2C9, but better human studies suggest it really doesn't affect drugs that use this enzyme. So this interaction is rated minor and unlikely. It's still worth mentioning to your pharmacist or doctor, especially since lesinurad already needs careful kidney monitoring, but you don't need to panic about it.

What Is Lesinurad Used For?

Brand name: Zurampic

  • Lesinurad is used in combination with a xanthine oxidase inhibitor to treat hyperuricemia (high levels of uric acid) in people with gout (sudden attacks of redness, swelling, pain, and heat in one or more joints) whose disease is not controlled with their current medication. Lesinurad is in a class of medications called selective uric acid reabsorption inhibitors. It works by helping the kidneys to remove uric acid from the body.
