Why this applies to Lusutrombopag

Lusutrombopag falls under P-GLYCOPROTEIN SUBSTRATES — the group Milk Thistle interacts with.

Theoretically, milk thistle might increase the absorption of P-glycoprotein substrates. However, this effect does not seem to be clinically significant.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.

Here's the short version: milk thistle can, in theory, block a transporter called P-glycoprotein that helps move Lusutrombopag (Mulpleta) in and out of your gut and cells. If that transporter is slowed, slightly more of the drug could get absorbed. With Lusutrombopag, that might mean a bit more platelet-boosting effect than expected. The honest truth, though, is that this concern is mostly theoretical, based on lab studies, and a careful human study with a similar drug showed no real change.

Since you'd typically take Lusutrombopag for just a short stretch before a procedure, this overlap is unlikely to cause trouble. Still, it's worth a quick mention to your pharmacist or doctor so they can confirm the timing is fine for you.

What Is Lusutrombopag Used For?

Brand name: Mulpleta

Mulpleta (lusutrombopag) is used to treat thrombocytopenia — a low platelet count — in adults who have chronic liver disease and are scheduled to have a procedure. Raising the platelet count before the procedure helps reduce the risk of bleeding.

Mulpleta is taken for a short, fixed course before the procedure. It is not meant to be used to bring platelet counts all the way to normal, or for long-term management of chronic liver disease.
