Why this applies to Nintedanib Esylate

Nintedanib Esylate falls under P-GLYCOPROTEIN SUBSTRATES — the group Milk Thistle interacts with.

Theoretically, milk thistle might increase the absorption of P-glycoprotein substrates. However, this effect does not seem to be clinically significant.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.

Here's the gist for Ofev (nintedanib): it relies on a transport pump called P-glycoprotein to move it in and out of cells. Milk thistle, in lab studies, can mildly slow that pump down. In theory, that could let a little more nintedanib get absorbed, which might nudge up its common side effects like nausea, diarrhea, stomach pain, or changes in liver tests.

The good news is the real-world evidence for this is weak. It comes mostly from test-tube work, and a human study with a similar pump drug (digoxin) showed no meaningful change. So for most folks this is unlikely to cause trouble. Still, since nintedanib can already be hard on the gut and liver, it's worth a quick chat with your pharmacist or doctor before adding milk thistle.

Nintedanib Esylate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist: milk thistle might slow down an enzyme called CYP3A4, which is one of the pathways your body uses to clear Ofev (nintedanib). If that enzyme gets blocked, a bit more nintedanib could hang around in your system, which in theory could nudge up its usual side effects, things like diarrhea, nausea, belly pain, lower appetite, or changes in liver blood tests.

That said, I don't want to overstate this. The evidence here is weak and conflicting, mostly lab and animal work, and human studies with milk thistle didn't show much real effect on similar drugs. The chance of a problem is low. Since nintedanib already taxes the liver, it's still worth a quick chat with your pharmacist or doctor before adding milk thistle, especially in concentrated supplement doses.

Milk thistle is a popular herbal supplement most often used for liver health, and its main active component is a group of compounds called silymarin. While it is generally well tolerated, the scientific evidence that it meaningfully improves liver disease or other conditions is mixed and far from conclusive. About Nintedanib

What Is Nintedanib Used For?

Brand name: Ofev

Nintedanib capsules are approved for adults with idiopathic pulmonary fibrosis (IPF) — a chronic, progressive scarring of the lungs — and for adults with chronic fibrosing interstitial lung diseases (ILDs) with a progressive phenotype, meaning fibrosing lung diseases that continue to worsen over time despite treatment.

Nintedanib capsules (including Ofev) are also indicated to slow the rate of decline in lung function in adults with systemic sclerosis-associated interstitial lung disease (SSc-ILD), a lung complication of the autoimmune condition systemic sclerosis (scleroderma). Note that the Nintedanib esylate capsule product is approved for IPF and chronic fibrosing ILDs with a progressive phenotype, but SSc-ILD approval is carried by Ofev and other nintedanib capsule products — always confirm with your prescriber which product you have been prescribed.
