Here's the short version on Annovera (the segesterone and ethinyl estradiol vaginal ring) with milk thistle. The concern on file is purely theoretical and comes from lab and animal work, not from people. In those studies, a compound in milk thistle called silymarin attached to one type of estrogen receptor (beta). The idea is that it might compete a little with the estrogen part of your ring, but this has never been shown to matter in real patients.

In plain terms, I wouldn't expect milk thistle to weaken your birth control or change how you feel. The rating here is minor and the evidence is weak. Still, since reliable contraception matters, it's worth a quick mention to your pharmacist or doctor so they know everything you're taking.

P-GLYCOPROTEIN SUBSTRATES

Segesterone Acetate, Ethinyl Estradiol also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, milk thistle might increase the absorption of P-glycoprotein substrates. However, this effect does not seem to be clinically significant.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.

If you're using the Annovera ring (segesterone and ethinyl estradiol) and thinking about milk thistle, here's the honest picture. In a test tube, milk thistle can slow down a transporter called P-glycoprotein that helps move certain drugs around your body. In theory, that could let a bit more hormone get absorbed. But this is mostly lab-based thinking, and a human study found milk thistle didn't change levels of digoxin, another drug handled by the same transporter.

So for your birth control, any real effect is considered minor and unlikely. Even if absorption nudged up slightly, you might notice nothing, or at most mild things like nausea, breast tenderness, or spotting. This isn't a reason to skip either one, but it's always smart to run your supplement list past your pharmacist or doctor first.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Annovera is a vaginal ring that releases two hormones, segesterone acetate and ethinyl estradiol, and your liver uses an enzyme called CYP3A4 to help clear them. The thought here is that milk thistle might slow that enzyme a little, which could in theory let estrogen levels run slightly higher and bump up side effects like nausea, breast tenderness, headaches, or mood changes.

Here's the honest part: this is a minor and unlikely concern. The lab studies disagree with each other, and careful human studies showed milk thistle did not meaningfully change how the body handles similar CYP3A4 drugs. So most people won't notice anything. Still, it never hurts to mention milk thistle to your pharmacist or doctor so they know your full list.
