Sildenafil Citrate falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the gist for Viagra (sildenafil) and milk thistle. Sildenafil is partly cleared by a liver enzyme called CYP2C9, and there's a theory that milk thistle might slow that enzyme down a bit. If that happened, sildenafil could hang around longer in your body, which could mean more of its usual effects like headache, facial flushing, a stuffy nose, or lightheadedness.

The good news is that the evidence here is weak and conflicting. Some lab studies hint at a slowdown, but better-quality human research suggests milk thistle doesn't meaningfully affect CYP2C9. This interaction is rated minor and unlikely. Still, if you take both and notice stronger side effects, give your pharmacist or doctor a quick heads-up.

While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist: your body clears sildenafil (Viagra) mainly through a liver enzyme called CYP3A4. In theory, if milk thistle slowed that enzyme down, sildenafil could hang around longer and its effects could feel stronger. That might mean more of the usual things people notice, like headache, facial flushing, a stuffy nose, or lightheadedness.

The good news is that the actual evidence here is weak. Real-world studies in people show milk thistle doesn't meaningfully change how the body handles CYP3A4 drugs, and any concern comes mostly from lab dishes and a few unusual case reports. So this is rated minor and unlikely. Still, if you take both regularly, it never hurts to mention it to your pharmacist or doctor.

Milk thistle is a popular herbal supplement most often used for liver health, and its main active component is a group of compounds called silymarin. What Is Sildenafil Used For?

Brand name: Viagra

Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.

Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
