Moringa and Abiraterone

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Abiraterone

Abiraterone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.

Theoretically, moringa might increase or decrease levels of CYP3A4 substrates.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the honest picture: your body uses a liver enzyme called CYP3A4 to help clear abiraterone, and moringa might nudge that enzyme one way or the other. The catch is that lab studies disagree. Some hint that moringa slows this enzyme (which could let abiraterone build up, possibly meaning more side effects like high blood pressure, fluid swelling, low potassium, or liver strain). Others suggest moringa speeds it up (which could weaken how well the drug controls your prostate cancer).

The good news is this is all based on test-tube and animal work, not real patient results, so we genuinely don't know if it matters in people. Food amounts in cooking are unlikely to be a problem, but concentrated supplements are less certain. Please run any moringa supplement past your oncology team before starting it.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

About Moringa

What is Moringa?

  • Nutritional supplement
  • Blood sugar support
  • Cholesterol support
  • Inflammation
  • Skin conditions

Moringa is a nutrient-rich plant whose leaves are widely used as a food and supplement, especially in parts of the world where malnutrition is common. Early research hints at possible benefits for blood sugar, cholesterol, and inflammation, but the human evidence is limited and not strong enough to recommend it as a treatment. Talk with your pharmacist or doctor before using moringa, especially if you are pregnant, breastfeeding, or taking medications.

About Abiraterone

What Is Abiraterone Used For?

Abiraterone acetate tablets are used to treat two types of advanced prostate cancer: metastatic castration-resistant prostate cancer (CRPC — cancer that has spread and keeps growing despite low testosterone levels) and metastatic high-risk castration-sensitive prostate cancer (CSPC — cancer that has spread but still responds to hormone-lowering treatment). It is always taken together with a corticosteroid — most products pair with prednisone, while Yonsa is specifically indicated with methylprednisolone.

All products — including Abiraterone, Abiraterone Acetate, Abirtega, Yonsa, and Zytiga — are taken by mouth as a tablet. Yonsa is approved only for metastatic CRPC, while the other products (Abiraterone, Abiraterone Acetate, Abirtega, and Zytiga) are approved for both metastatic CRPC and metastatic high-risk CSPC.
