Theoretically, using kava with hepatotoxic drugs might increase the risk of liver damage.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the concern with pairing kava and abiraterone. Both can be tough on the liver. Abiraterone itself can raise your liver enzymes, which is why your doctor checks blood work regularly while you're on it. Kava has been tied to over 100 reported cases of liver injury, mostly with heavy or long-term use. Stacking the two together could, in theory, add more strain on your liver.

To be honest, this is based mostly on anecdotal reports, not strong studies, so it isn't a guaranteed problem. Still, with a medication like abiraterone where liver health really matters, I'd play it safe. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and please check with your pharmacist or oncologist before using kava.

Abiraterone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version on combining kava with abiraterone (Zytiga): the worry is purely about how your liver processes this medication. Abiraterone is handled in part by a liver enzyme called CYP3A4. Early lab (test tube) studies hinted that kava might slow that enzyme down, which in theory could let abiraterone build up a bit. But that's where it stays, in theory.

When researchers actually tested kava in real people for 28 days, it did not meaningfully change CYP3A4 activity. So this is a low-priority, unlikely interaction. The bigger practical concern is that both kava and abiraterone can stress the liver, so I'd still loop in your oncologist or pharmacist before adding kava, especially since your liver gets monitored during treatment.

Kava is a Pacific Island plant traditionally used to promote relaxation and ease anxiety, and some studies suggest it may help mild anxiety. However, kava has been linked to rare but serious liver damage, so it should be used with caution, avoided by people with liver problems, and only used after talking with your pharmacist or doctor.

About Abiraterone

What Is Abiraterone Used For?

Abiraterone acetate tablets are used to treat two types of advanced prostate cancer: metastatic castration-resistant prostate cancer (CRPC — cancer that has spread and keeps growing despite low testosterone levels) and metastatic high-risk castration-sensitive prostate cancer (CSPC — cancer that has spread but still responds to hormone-lowering treatment). It is always taken together with a corticosteroid — most products pair with prednisone, while Yonsa is specifically indicated with methylprednisolone.

All products — including Abiraterone, Abiraterone Acetate, Abirtega, Yonsa, and Zytiga — are taken by mouth as a tablet. Yonsa is approved only for metastatic CRPC, while the other products (Abiraterone, Abiraterone Acetate, Abirtega, and Zytiga) are approved for both metastatic CRPC and metastatic high-risk CSPC.
