Why this applies to Pacritinib

Pacritinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.

Theoretically, moringa might increase or decrease levels of CYP3A4 substrates.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.

Pacritinib (Vonjo), the medication you take for myelofibrosis, is handled in part by a liver enzyme called CYP3A4. Moringa might nudge that enzyme in either direction, which is why this pairing gets flagged. If moringa slowed the enzyme, pacritinib could build up and you might notice more of its usual side effects, like diarrhea, nausea, low platelets, or bleeding. If moringa sped the enzyme up instead, pacritinib levels could drop and work less well.

Here's the honest part: this is mostly lab-dish research that points both ways, and one human study showed no real change. So the practical risk is likely small, especially with food amounts. Still, since pacritinib is a serious drug, please check with your pharmacist or doctor before adding moringa supplements.

What Is Pacritinib Used For?

Brand name: Vonjo

Vonjo (pacritinib) is used to treat adults with intermediate or high-risk myelofibrosis (MF) — a serious bone marrow disease — including primary MF and secondary MF that developed after polycythemia vera or essential thrombocythemia (other blood disorders). It is specifically approved for patients whose platelet count is below 50 × 10⁹/L (very low platelets), a group that has limited treatment options.

This approval is based on demonstrated reductions in spleen size. Continued approval may depend on results from confirmatory trials showing broader clinical benefit.
