Moringa and Ethiny Estradiol, Levonorgestrel

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Moringa interacts with whole categories of medication. Ethiny Estradiol, Levonorgestrel falls into 3 of those categories, so it can interact with Moringa in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

1 of 3

Why this applies to Ethiny Estradiol, Levonorgestrel

Ethiny Estradiol, Levonorgestrel falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.

Theoretically, moringa might increase or decrease levels of CYP3A4 substrates.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Your birth control pill contains two hormones, ethinyl estradiol and levonorgestrel, and your liver clears them partly through an enzyme called CYP3A4. The concern with moringa is that, in test-tube studies, it has pulled in both directions. Some lab work suggests moringa slows that enzyme down (which could let hormone levels creep up a bit), while other lab work suggests it speeds the enzyme up (which could lower hormone levels and, in theory, make the pill slightly less reliable).

Here is the reassuring part: this is only laboratory evidence, and a human study using moringa leaf powder didn't change levels of another CYP3A4 drug. Everyday food amounts are unlikely to matter. Still, since pregnancy prevention is at stake, mention any concentrated moringa supplement to your pharmacist or doctor first.

P-GLYCOPROTEIN SUBSTRATES

2 of 3

Ethiny Estradiol, Levonorgestrel also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, moringa leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that moringa leaf extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with your birth control pill (Microgynon, Logynon) and moringa. In a lab dish, moringa leaf extract slowed down a transporter called P-glycoprotein, which helps move certain medicines around and out of the body. In theory, blocking that pump could let a little more of the hormones build up. This is not a prodrug, so more drug would mean a slightly stronger hormone effect, not a weaker one.

But be reassured: this has only been seen in test-tube studies, never in actual people, so it's very theoretical. Food amounts of moringa in cooking are unlikely to matter. If you're using concentrated moringa supplements daily, just mention it to your pharmacist or doctor so they can keep an eye on things.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

3 of 3

Ethiny Estradiol, Levonorgestrel also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, moringa might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that moringa extract induces CYP1A2 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version on combining moringa with your birth control pill (the ethinyl estradiol and levonorgestrel combo found in Logynon and Microgynon). Some lab studies suggest moringa might speed up a liver enzyme called CYP1A2, which plays a small part in clearing estrogen from your body. In theory, that could lower the hormone levels a bit. With a contraceptive, anything that weakens the hormones is worth noting, since the worry would be reduced reliability or some breakthrough spotting.

That said, this is very early evidence, only from test-tube and animal work, and rated minor. Food amounts of moringa in cooking are unlikely to cause trouble. If you take concentrated moringa supplements regularly, just mention it to your pharmacist or doctor so they can reassure you.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

About Moringa

What is Moringa?

  • Nutritional supplement
  • Blood sugar support
  • Cholesterol support
  • Inflammation
  • Skin conditions

Moringa is a nutrient-rich plant whose leaves are widely used as a food and supplement, especially in parts of the world where malnutrition is common. Early research hints at possible benefits for blood sugar, cholesterol, and inflammation, but the human evidence is limited and not strong enough to recommend it as a treatment. Talk with your pharmacist or doctor before using moringa, especially if you are pregnant, breastfeeding, or taking medications.
