Why this applies to Thiotepa

Thiotepa falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.

Theoretically, moringa might increase or decrease levels of CYP3A4 substrates.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.

Here's something worth knowing about thiotepa: it's actually a prodrug. Your liver uses an enzyme called CYP3A4 to turn thiotepa into its active form (called TEPA), which is the part that fights the cancer. So this enzyme matters a lot for how well the drug works and how much side effect you get.

The catch is that lab studies disagree on what moringa does to CYP3A4. Some suggest it slows the enzyme (which could mean less active drug and possibly weaker treatment), while others suggest it speeds it up (which could mean more active drug and harsher effects like low blood counts, nausea, or infection risk). This is only test-tube evidence, not proven in people, but with a chemotherapy drug there's no room to guess. Please clear any moringa supplement with your oncology team first.

About Thiotepa Injection

What Is Thiotepa Injection Used For?

Brand names: Thioplex, Tepadina

Tepadina (thiotepa) injection is used for several purposes. It treats adenocarcinoma (a type of cancer) of the breast or ovary. It is also used to control malignant effusions — abnormal fluid buildup in body cavities caused by cancer spreading to those areas. For bladder cancer, Tepadina treats superficial papillary carcinoma of the urinary bladder.

Tepadina is also used in pediatric patients with class 3 beta-thalassemia (a severe inherited blood disorder). In those patients, it is given as part of a preparative regimen — a conditioning treatment to reduce the risk of graft rejection before an allogeneic stem cell transplant (a transplant using donor stem cells), alongside the chemotherapy drugs busulfan and cyclophosphamide.
