Axitinib falls under Cytochrome P450 3A4 (Cyp3A4) Substrates — the group Puravive interacts with.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the short version for Inlyta (axitinib): your liver uses an enzyme called CYP3A4 to clear this drug out of your body. Puravive contains Panax ginseng, which has shown mixed effects on that same enzyme. In some studies it slowed CYP3A4 down (which could let axitinib build up, raising the risk of side effects like high blood pressure, fatigue, diarrhea, hand-foot skin reactions, or liver irritation). In others it sped the enzyme up (which could lower axitinib levels and make it work less well against the cancer).

Honestly, the evidence here is weak and contradictory, so we just don't know which way it would go. Because axitinib is a serious cancer medicine, I'd play it safe and check with your oncologist or pharmacist before adding Puravive. Don't change your axitinib on your own.

What Is Axitinib Used For?

Brand name: Inlyta

Axitinib (brand name Inlyta) is used to treat advanced renal cell carcinoma (RCC), a type of kidney cancer. Inlyta can be used as a first-line treatment — meaning the very first treatment you receive — when combined with either avelumab or pembrolizumab, two other cancer medicines.

Inlyta is also approved as a single agent (on its own) for patients with advanced RCC who have already tried one other systemic (body-wide) cancer therapy that did not work or stopped working.
