Why this applies to Treprostinil

Treprostinil falls under Anticoagulant/Antiplatelet Drugs — the group Puravive interacts with.

Puravive, a dietary supplement, contains several ingredients that may interact with anticoagulant drugs due to their potential mild anticoagulant effects. These ingredients include Kudzu, Panax ginseng, Phellodendron, and Propolis. Ginsenosides in Panax ginseng may decrease platelet aggregation in lab studies, but human studies have not confirmed this effect. Animal studies suggest low oral bioavailability of certain compounds in ginseng, which could explain differences between lab and human research. Propolis water extract and caffeic acid phenethyl ester, found in propolis, have also been shown to inhibit platelet aggregation in lab studies. Additionally, in an animal model, propolis taken with warfarin reduced the effectiveness of warfarin, suggesting a potential interaction with anticoagulant drugs. Lastly, Phellodendron contains berberine, which studies show may inhibit platelet aggregation.

Here's something a lot of people don't realize about treprostinil (Orenitram): besides relaxing blood vessels in your lungs, it also makes your platelets a little less likely to stick together. In other words, it has a mild blood-thinning side to it on its own.

Now, a few ingredients in Puravive (like Panax ginseng, propolis, and the berberine in Phellodendron) have shown a similar platelet-slowing effect, mostly in lab dishes rather than solid human studies. So in theory, stacking them with treprostinil could add up and nudge your bleeding risk higher. That might look like easier bruising, nosebleeds, or bleeding gums. The evidence here is weak, so don't panic, but please run Puravive by your pharmacist or doctor before starting it.

Puravive contains quercetin, which has been found to slightly reduce blood pressure in individuals with mild hypertension (https://pubmed.ncbi.nlm.nih.gov/17951477/). There is a theoretical possibility that it could enhance the blood pressure-lowering effects of antihypertensive medications.

Here's the simple version. Treprostinil (Orenitram) widens blood vessels to ease the strain in your lungs, and one of its normal effects is lowering blood pressure. Puravive contains quercetin, which in one small study nudged blood pressure down a little in people with mild high blood pressure. So if you took both, there's a theoretical chance the blood-pressure-lowering could stack up a bit.

What might that feel like? Possibly more dizziness, lightheadedness when you stand, flushing, or headache, which are already common with treprostinil. To be clear, this is rated minor and the evidence is weak, so for most people it's unlikely to be a big deal. Still, since treprostinil is a serious medication, please run Puravive past your pharmacist or doctor before starting it.

About Treprostinil

What Is Treprostinil Used For?

Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.

Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
