Higher doses of tianeptine have opioid like effects and may have additive effects with other sedative drugs. In one case report, a 42-year-old male presented with respiratory depression and aspiration pneumonia due to concomitant use of high-dose tianeptine and alprazolam.

The piece of this combo product I want to focus on is the codeine. Codeine is an opioid pain reliever and cough suppressant, and like other opioids it can slow your breathing and make you drowsy. Tianeptine, especially at the high doses found in those gas station products, can act a lot like an opioid itself. Stack the two together and the sedating, breathing-slowing effects can add up.

That's the real worry here: more than expected sleepiness, foggy thinking, slowed breathing, and in serious cases the kind of respiratory depression seen in a reported case with high-dose tianeptine. This concern is based on how these drugs work rather than hard studies, but the risk is real enough that I'd talk it over with your pharmacist or doctor before mixing them.

Chronic use of aspirin might increase the levels and clinical effects of tianeptine.
Aspirin is converted to salicylic acid in the body. In vitro research shows that salicylic acid binds to the same protein sites as tianeptine, displacing protein-bound tianeptine and leading to higher levels of tianeptine in the blood.

The piece of this combo that matters here is the aspirin. When you take aspirin regularly, your body turns it into salicylic acid, and lab studies suggest that salicylic acid can bump tianeptine off the proteins it normally rides on in your blood. That could leave more free, active tianeptine floating around, nudging its effects (and side effects) upward. Since tianeptine can behave like an opioid at higher amounts, more in your system isn't something to take lightly.

That said, this is based only on test-tube research, so the real-world impact in people isn't well proven. I'll be honest with you: my bigger concern is the tianeptine itself, since it isn't FDA-approved and carries real risks of dependence. Please talk with your pharmacist or doctor before mixing these.
