The most common side effects reported when taken orally are abdominal discomfort, bloating, diarrhea, nausea, and vomiting. Rare but serious effects include shiitake dermatitis (a distinctive skin eruption) from consuming raw mushroom, and in people who handle shiitake extensively, allergic reactions such as asthma, contact dermatitis, and in some cases hypersensitivity pneumonitis (a lung condition).

Theoretically, shiitake mushroom might decrease levels of drugs metabolized by CYP2D6.
In vitro studies suggest that the shiitake mushroom extract AHCC might induce the CYP2D6 enzyme. This effect has not been reported in humans.
Immunosuppressants
Theoretically, taking shiitake mushroom might decrease the effects of immunosuppressive therapy.
In vitro evidence suggests that shiitake mushroom extracts stimulate immune function.
