Andrographis is generally well tolerated in the short term, though long-term safety and product quality aren't well established. The most common side effects — abdominal discomfort, altered taste, diarrhea, dizziness, fatigue, headache, nausea, vomiting, itching, rash, and hives — tend to show up at higher doses (5–10 mg/kg of the active compound andrographolide) or after more than 14 days of use.

Rare serious reactions, including severe allergies and anaphylaxis, have been reported. Skin reactions are frequent and can range from mild rashes to Stevens-Johnson syndrome or other severe skin conditions.

Avoid this product during pregnancy due to animal study concerns about reproductive effects. Don't use it while breastfeeding — there isn't enough safety data.

However, this reduced bioavailability is not correlated with a reduction in the anti-inflammatory effects of etoricoxib in arthritic mice models. The clinical significance of this interaction is unclear.
