Neem leaf preparations appear well tolerated in short-term use for most adults, though rigorous safety studies are limited. The main concern with oral neem is dose and form—neem oil and seed extracts are toxic if swallowed, and children are especially at risk for serious effects like anemia and metabolic imbalance.

Topically, neem is generally well tolerated; the most common side effect is contact dermatitis in sensitive skin. Rare but serious oral side effects in adults have included heart rhythm problems and kidney damage.
