Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the gist with Tagrisso (osimertinib) and uva ursi. Your liver uses an enzyme called CYP3A4 to help clear osimertinib from your body. In lab dish studies, uva ursi seems to slow that enzyme down. If that happened in a real person, osimertinib could build up a bit higher than expected, which in theory could mean more of its usual side effects, things like diarrhea, dry or sore skin, mouth sores, or nail changes.

The honest part: this is only seen in test-tube research, not in actual patients, so it's a theoretical concern rather than a proven problem. Still, since osimertinib is a serious cancer medicine, I wouldn't add uva ursi without checking with your oncologist or pharmacist first.

Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the gist for Tagrisso (osimertinib) and uva ursi. Osimertinib is partly moved around your body by a transporter pump called P-glycoprotein. In a lab dish, uva ursi seemed to slow that pump down. If that happened in people, slightly more osimertinib could stay in your system, which in theory could nudge up side effects like diarrhea, mouth sores, rash, dry skin, or fatigue. Osimertinib is an active drug, not a prodrug, so this would point toward a bit more drug effect, not less.

Honestly though, this is only test-tube data and has never been shown in humans, so I'd file it as a small theoretical concern. Since uva ursi can also stress the liver, please run it by your oncologist or pharmacist before starting it.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. Evidence in people is limited, and because high doses or long-term use may be harmful to the liver and could be toxic, it should only be used short-term and with guidance from a healthcare professional. Avoid it during pregnancy and breastfeeding.

About Osimertinib

What Is Osimertinib Used For?

Brand name: Tagrisso

Tagrisso is used to treat adults with non-small cell lung cancer (NSCLC) whose tumors have specific EGFR mutations — either an exon 19 deletion or an exon 21 L858R mutation — confirmed by an FDA-approved test. It is used in several settings: after surgery to reduce the risk of cancer returning (adjuvant therapy), for locally advanced stage III NSCLC that has not progressed after chemoradiation, and as a first-line treatment for locally advanced or metastatic NSCLC (including in combination with pemetrexed and platinum-based chemotherapy).

Tagrisso is also approved for adults with metastatic NSCLC that carries a specific resistance mutation called T790M, when the cancer has stopped responding to another EGFR-targeted therapy.
