CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Sirolimus (albumin bound) falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Sirolimus (Fyarro) is cleared from your body mainly by a liver enzyme called CYP3A4. In lab studies, uva ursi has shown a tendency to slow that same enzyme down. If that happened in a real person, your body could clear sirolimus more slowly, letting levels creep up. Higher levels can mean more side effects like mouth sores, low blood counts, infections, swelling, high blood sugar, or high cholesterol.
Here's the honest part: this concern comes only from test-tube data, not from people, so it's theoretical at this point. Still, sirolimus is a drug where small changes in blood level matter, and it's used for a serious condition. Please don't start or stop uva ursi on your own. Check with your oncology team or pharmacist first.

P-GLYCOPROTEIN SUBSTRATES

In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Fyarro (albumin-bound sirolimus). This medication is moved around the body by a transporter protein called P-glycoprotein. In lab dish studies, uva ursi seemed to block that transporter. If that happened in people, it could theoretically let a bit more sirolimus stick around, which might nudge up side effects like mouth sores, fatigue, low blood counts, swelling, or a higher infection risk.
The honest part: this is only seen in test-tube work, never confirmed in humans, and it's rated a minor interaction. So I wouldn't lose sleep over it. Still, sirolimus is a serious cancer medicine, so please check with your oncologist or pharmacist before adding uva ursi rather than starting or stopping anything on your own.
