Why this applies to Telaprevir

Telaprevir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the short version: your liver uses an enzyme called CYP3A4 to help break down telaprevir (Incivek). In a lab dish, uva ursi seemed to slow that same enzyme down. If that were to happen in real life, your body might clear telaprevir a little more slowly, so more of it could build up. That could mean a higher chance of telaprevir's usual side effects, like rash and itching, nausea, or anemia.

I want to be honest, though: this has only been seen in test-tube studies, never in actual people, so it's more of a theoretical concern than a proven one. Uva ursi also isn't meant for long-term use. Before combining the two, please check with your pharmacist or doctor so they can keep an eye on things.

Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the short version: telaprevir is moved around your body by a transporter called P-glycoprotein (P-gp), which acts like a little pump that helps shuttle the drug in and out of cells. In lab dish studies, uva ursi seemed to slow that pump down. If that happened in a real person, telaprevir could build up a bit more than expected, which in theory might mean stronger side effects like nausea, fatigue, itching, anal discomfort, or rash.

The important thing to know is that this is theoretical. It has only been seen in test-tube studies, never confirmed in people, and it's rated a minor concern. Still, since uva ursi can stress the liver and you'd be treating a liver condition, I'd run it past your pharmacist or doctor before combining them.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. Evidence in people is limited, and because high doses or long-term use may be harmful to the liver and could be toxic, it should only be used short-term and with guidance from a healthcare professional. Avoid it during pregnancy and breastfeeding.

About Telaprevir

What Is Telaprevir Used For?

Brand name: Incivek

  • Telaprevir is used along with two other medications (ribavirin [Copegus, Rebetol] and peginterferon alfa [Pegasys]) to treat chronic hepatitis C (an ongoing viral infection that damages the liver) in people who have not yet been treated for this condition or whose condition could not successfully be treated with ribavirin and peginterferon alfa alone. Telaprevir is in a class of medications called protease inhibitors. It works by decreasing the amount of hepatitis C virus (HCV) in the body. Telaprevir may not prevent the spread of hepatitis C to other people.
