Glimepiride falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafinil might increase the levels and clinical effects of CYP2C9 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to reduce CYP2C9 expression in vitro.

Here's the gist: your body relies on a liver enzyme called CYP2C9 to clear glimepiride out of your system. Adrafinil turns into modafinil in the liver, and lab studies hint that modafinil may slow this same enzyme down. If that happens, glimepiride could linger longer and hit a little harder than expected.

Since glimepiride lowers blood sugar by prompting your pancreas to release insulin, stronger or longer effects could mean low blood sugar (hypoglycemia), with symptoms like shakiness, sweating, dizziness, confusion, or feeling unusually hungry. That said, this is only a theoretical concern from test-tube data, not real patient reports. Still, if you use both, check your blood sugar more closely and talk with your pharmacist or doctor first.

What Is Glimepiride Used For?

Brand name: Amaryl

Glimepiride tablets are used to improve blood sugar control in adults with type 2 diabetes. It is always used together with a healthy diet and regular exercise — it is not a replacement for those habits.

Glimepiride is not intended for type 1 diabetes or diabetic ketoacidosis (a serious complication where the body produces high levels of blood acids called ketones). It would not be effective in those situations.
