Propranolol Hydrochloride falls under ANTIHYPERTENSIVE DRUGS — the group Adrafinil interacts with.

Theoretically, adrafinil might reduce the effects of antihypertensive drugs.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to increase diastolic and systolic blood pressure. Retrospective analyses show that patients taking modafinil had a small but increased rate of requiring new antihypertensive drugs or increased use of antihypertensive drugs when compared with placebo.

Here's the gist: once your liver turns adrafinil into modafinil, that active form can nudge blood pressure and heart rate upward. Propranolol is a beta blocker working in the opposite direction (it calms the heart and relaxes blood vessels), so adrafinil could partly push back against what propranolol is trying to do. In theory, that might mean propranolol doesn't control blood pressure or heart rate as well as expected.

I'll be honest, this is a theoretical concern based on how these drugs behave, not strong real-world proof, and any effect is likely small. Still, adrafinil is unregulated and hard on the liver. One special note: the propranolol solution (Hemangeol) is given to infants for hemangiomas, and adrafinil is not appropriate for them. Please check with your doctor or pharmacist before combining anything.

Here's the gist with adrafinil and propranolol. Once your liver turns adrafinil into modafinil, that modafinil may slow down an enzyme called CYP2C19, which is one of the pathways your body uses to clear propranolol. If that pathway slows, propranolol could build up a bit more than usual, nudging its effects stronger. With a beta blocker like this, that might show up as a slower heart rate, lower blood pressure, lightheadedness, or feeling unusually tired.

I want to be honest about the strength of this: it's theoretical, based only on lab data, not real patient reports. It also matters that propranolol here (Hemangeol) is dosed for infants, so any change should never be made on your own. Please run this by your child's doctor or pharmacist first.

Here's the situation with these two. Your liver uses an enzyme called CYP1A2 to help clear propranolol from your body. Adrafinil turns into modafinil inside you, and modafinil may slightly speed up that enzyme. In theory, that could clear propranolol a little faster and make it work less well.

For an infant being treated for a hemangioma, weaker propranolol might mean the growth doesn't shrink as expected. I'll be honest though: this concern is theoretical, based only on lab data, and the effect appears weak. On top of that, adrafinil itself isn't something you'd give an infant. If you have any questions about propranolol for your child, please check with your doctor or pharmacist before changing anything.

What Is Propranolol (Infantile Hemangioma) Used For?

Brand name: Hemangeol

This intravenous (IV) form of propranolol is used in hospital settings to treat serious, life-threatening heart rhythm problems. It works on several types of abnormal rhythms, including those that happen during anesthesia or are caused by drug toxicity.

Specific uses include fast heart rhythms coming from the upper chambers of the heart (supraventricular tachycardia), dangerous rhythms caused by too much digitalis in the body, and certain fast rhythms triggered by excess stress hormones (catecholamines) during surgery.
