Simvastatin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the gist for you. Your body clears simvastatin using a liver enzyme called CYP3A4. Some lab research hints that cascara sagrada might gently speed that enzyme up. If that happened in real life, your simvastatin could get broken down a little faster, which could make it slightly less effective at lowering your bad (LDL) cholesterol.

Now, the honest part: this comes only from test-tube studies, and even there the effect was far weaker than known strong enzyme boosters. So I wouldn't lose sleep over it. The bigger practical issue with cascara is that it's a harsh, short-term laxative that can throw off your fluids and electrolytes. Before adding it, just run it by your pharmacist or doctor so they can check it fits your situation.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

About Simvastatin

What Is Simvastatin Used For?

Brand name: Zocor

Simvastatin tablets are used alongside a cholesterol-lowering diet to treat high LDL cholesterol (bad cholesterol), high triglycerides, and certain inherited cholesterol disorders, including heterozygous familial hypercholesterolemia (HeFH) in adults and children aged 10 and older, and homozygous familial hypercholesterolemia (HoFH) in adults.

In adults who already have coronary heart disease, cerebrovascular disease, peripheral vascular disease, or diabetes and are at high risk for heart events, simvastatin is also used to reduce the risk of death from heart disease, non-fatal heart attack, stroke, and the need for procedures to open blocked arteries. Zocor is one brand-name product containing simvastatin and carries these same indications.
