Why this applies to Copanlisib

Copanlisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the short version for Copanlisib (Aliqopa): your body clears this medicine partly through a liver enzyme called CYP3A4. A compound in Ceylon Leadwort called plumbagin has been shown, in test-tube studies only, to slow that same enzyme down. If that effect carried over into people, copanlisib could build up more than expected, which might mean stronger side effects like high blood sugar, high blood pressure, low white blood cell counts, infections, or diarrhea.

That said, this is theoretical right now and has not been seen in actual patients. Since copanlisib is a serious cancer treatment, please don't add Ceylon Leadwort without first checking with your oncology team or pharmacist.

Plumbagin, a constituent of Ceylon leadwort, has demonstrated immunosuppressant activity in animal research. Theoretically, Ceylon leadwort might interfere with immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and others.

Here's the honest picture for copanlisib (Aliqopa). The concern on file is theoretical and comes only from lab and animal studies. In those studies, plumbagin (a compound in Ceylon Leadwort) showed its own immune-calming activity. The worry is that adding it on top of a drug that already affects the immune system could muddy the waters.

Copanlisib isn't a classic immunosuppressant like the ones listed, it's a targeted cancer drug for follicular lymphoma. We don't have real human data showing a true problem here, so I wouldn't panic. Still, while you're on a serious cancer therapy, I'd skip concentrated Ceylon Leadwort supplements until your oncology team or pharmacist signs off. Please loop them in before adding anything new.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts.
