Why this applies to Darunavir, Cobicistat

Darunavir, Cobicistat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the deal with Prezcobix (darunavir plus cobicistat) and Ceylon Leadwort. Darunavir is broken down by an enzyme called CYP3A4, and the cobicistat in this combo actually works by slowing that same enzyme down on purpose, which keeps darunavir levels high enough to fight HIV. In lab studies, a compound in Ceylon Leadwort called plumbagin also blocks CYP3A4. So in theory, adding it on top could push darunavir levels even higher, which might mean more side effects like nausea, diarrhea, rash, or headache.

I want to be honest though: this is only seen in test-tube research, not in real people, so we can't say how big a deal it really is. Since HIV therapy is something you don't want to disturb, please run this by your pharmacist or doctor before mixing the two.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. What Is Darunavir and Cobicistat Used For?

Brand name: Prezcobix

Prezcobix and Prezcobix Ped are used to treat HIV-1 (human immunodeficiency virus type 1) infection. They are always used together with other antiretroviral (anti-HIV) medicines — never alone. Both products are approved for people who are new to HIV treatment (treatment-naïve) and for those who have taken HIV medicines before (treatment-experienced), as long as certain darunavir resistance mutations are not present.

Prezcobix tablets are approved for adults and pediatric patients weighing at least 40 kg. Prezcobix Ped tablets for oral suspension extend coverage to pediatric patients 3 years of age and older weighing at least 15 kg.
