Dextromethorphan HBr, Guaifenesin falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the gist for your cough syrup: the dextromethorphan part is cleared from your body mainly by a liver enzyme called CYP2D6. Plumbagin, a compound in Ceylon Leadwort, has been shown to slow that same enzyme down in lab dishes. If that held true in people, dextromethorphan could build up a bit, which might mean more drowsiness, dizziness, or feeling foggy. The guaifenesin part isn't really involved here.

I want to be honest about how soft this evidence is. It comes only from test-tube work, not from real people, so we genuinely don't know if it matters in everyday use. Still, if you plan to use a concentrated Leadwort supplement, run it by your pharmacist or doctor first.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the simple version. Plumbagin, the main active compound in Ceylon Leadwort, slowed down an enzyme called CYP3A4 in lab dish studies. That enzyme is one of the helpers your liver uses to clear dextromethorphan (the cough suppressant in this combo). If that pathway gets blocked, dextromethorphan could build up a bit more than usual, which might mean more drowsiness, dizziness, or feeling foggy. The guaifenesin part isn't really affected this way.

I want to keep this in perspective, though. This was only seen in test tubes, never reported in actual people, so it's theoretical at this point. Still, if you want to use Ceylon Leadwort while taking a cough product, just run it by your pharmacist or doctor first to be safe.

Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts.
