Dextromethorphan, Pseudoephedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the gist for your Sudafed Cold/Flu, which pairs dextromethorphan (the cough suppressant) with pseudoephedrine (the decongestant). Dextromethorphan is cleared mainly by a liver enzyme called CYP2D6. In test-tube studies, plumbagin from Ceylon Leadwort slowed that same enzyme down. If that holds true in people, dextromethorphan could build up and hang around longer, which might mean more dizziness, drowsiness, or a foggy, spacey feeling.

I want to be honest: this is theoretical, seen only in lab work and never reported in real patients, so I wouldn't lose sleep over it. Still, if you're using Ceylon Leadwort regularly, mention it to your pharmacist or doctor first so they can keep an eye on how you're feeling.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist with Sudafed Cold/Flu, which combines dextromethorphan (a cough suppressant) and pseudoephedrine (a decongestant). A compound in Ceylon Leadwort called plumbagin has slowed down a liver enzyme (CYP3A4) in lab dishes. CYP3A4 is one of the routes that helps clear dextromethorphan. If that enzyme were blocked, dextromethorphan could build up a bit, which might mean more drowsiness, dizziness, or feeling foggy.

That said, this is only seen in test-tube research, never confirmed in real people, so I wouldn't lose sleep over it. Still, since both ingredients can already cause jitteriness or sleep trouble, it's worth a quick chat with your pharmacist before combining them.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts.
