Etanercept-SZZS falls under IMMUNOSUPPRESSANTS — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has demonstrated immunosuppressant activity in animal research. Theoretically, Ceylon leadwort might interfere with immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and others.

Erelzi (a biosimilar to Enbrel) is a TNF-blocker. It calms an overactive immune system to ease conditions like rheumatoid arthritis, psoriasis, or ankylosing spondylitis. Ceylon Leadwort's main compound, plumbagin, has shown its own immune-dampening effects, but only in lab dishes and animal studies, not in people taking medications like this one.

So this is a theoretical concern, not something proven in patients. The worry is that adding another agent that nudges the immune system could, in theory, add to the overall immune-lowering effect, which might raise infection risk. We really don't have human data to say how much, if any, that matters. Since the evidence is thin and Erelzi is a serious drug, please run this by your pharmacist or doctor before combining them.
