Why this applies to Etrasimod

Etrasimod falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the short version: your body partly relies on an enzyme called CYP2C9 to help clear Etrasimod (Velsipity). A compound in Ceylon Leadwort called plumbagin has been shown to slow that enzyme down, but so far only in lab dish studies, not in real people. Etrasimod is an active drug (not a prodrug), so if that enzyme is blocked, the medicine could build up a bit more than expected.

In theory, higher Etrasimod levels could mean more of its known effects, like a slower heart rate early on, headache, or changes in blood pressure and liver labs. The key word is theoretical, the evidence is weak. Still, before adding this supplement, please run it by your pharmacist or doctor so they can weigh in for your situation.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the situation with these two. Etrasimod (Velsipity) is used to treat ulcerative colitis, and your body relies partly on an enzyme called CYP3A4 to break it down. Ceylon Leadwort contains plumbagin, which in lab dish studies has slowed that same enzyme. If that happened in people, etrasimod could clear more slowly and build up a bit, which might make its effects and side effects stronger, think slower heart rate, headache, dizziness, or a bigger drop in your body's infection-fighting cells.

The important caveat: this is only seen in test-tube research, never actually reported in humans, so it stays theoretical for now. Still, since etrasimod already affects heart rate and immunity, I'd play it safe and check with your pharmacist or doctor before combining them.

Plumbagin, a constituent of Ceylon leadwort, has demonstrated immunosuppressant activity in animal research. Theoretically, Ceylon leadwort might interfere with immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and others.

Here's the situation with Etrasimod (Velsipity), a medicine that calms an overactive immune system to treat ulcerative colitis. Ceylon Leadwort contains plumbagin, and in lab and animal research only, plumbagin has shown some immune-dampening effects of its own. The concern is that adding it on top of Etrasimod could stack immune suppression, theoretically raising your chance of infections, or it could tug on your immune balance in an unpredictable way.

I want to be honest: this is a theoretical worry based on weak, non-human evidence, not real cases in people. Still, since Etrasimod already lowers your immune defenses, I wouldn't combine the two without checking first. Please run it by your pharmacist or gastroenterologist before starting Ceylon Leadwort.
