Why this applies to Fexofenadine

Fexofenadine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's some good news if you take Allegra (fexofenadine) and are curious about Ceylon Leadwort. The concern on file is that plumbagin, a compound in this plant, may block an enzyme called CYP3A4 in lab dishes. In theory, blocking that enzyme could let certain drugs build up. But fexofenadine isn't really one of those drugs. It's barely broken down by your liver at all, and instead leaves your body mostly unchanged through transporters in your gut and kidneys, so the CYP3A4 angle doesn't apply much here.

On top of that, this interaction has only been seen in test-tube studies, never in actual people. So I wouldn't lose sleep over it. Fexofenadine is also a very mild drug. Still, since Ceylon Leadwort itself can be irritating, please run it by your pharmacist or doctor before starting.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. What Is Fexofenadine Used For?

Brand name: Allegra

Fexofenadine temporarily relieves symptoms of hay fever (seasonal allergic rhinitis) and other upper respiratory allergies, including runny nose, sneezing, itchy or watery eyes, and itching of the nose or throat.

Several products — including Allegra Hives 24hr tablets — are specifically used to reduce hives (urticaria) and relieve the itching that comes with them. Keep in mind that fexofenadine will not prevent hives or stop an allergic skin reaction from happening in the first place.
