Why this applies to Leniolisib

Leniolisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the short version for anyone taking Leniolisib (Joenja) and thinking about Ceylon Leadwort. Your body clears leniolisib largely through a liver enzyme called CYP3A4. In test-tube studies, plumbagin (the active compound in Ceylon Leadwort) appears to slow that same enzyme down. If it did the same thing in people, your leniolisib could clear more slowly and build up, which may mean more side effects like headache, diarrhea, nausea, or changes in blood counts.

I want to be honest about the evidence though: this is only lab data, and it has not been shown to happen in real patients. So it's a theoretical concern, not a proven one. Since leniolisib treats a serious immune condition, please don't add Ceylon Leadwort without checking with your doctor or pharmacist first.

What Is Leniolisib Used For?

Brand name: Joenja

Joenja (leniolisib) is approved to treat activated phosphoinositide 3-kinase delta syndrome, commonly called APDS — a rare genetic immune disorder in which certain immune cells (B and T cells) don't work properly. It is approved for adults and pediatric patients who are at least 12 years old.

APDS is caused by gene variants that make a specific enzyme (PI3K-delta) overactive, leading to immune system dysfunction. Joenja is the brand name for leniolisib in tablet form, taken by mouth.
