Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Zokinvy (lonafarnib). A compound in Ceylon Leadwort called plumbagin can slow down certain liver enzymes that help clear medications. Lonafarnib is one of those drugs that depends on the liver to break it down, so in theory, pairing the two could let lonafarnib build up to higher levels than intended. That matters because lonafarnib already tends to cause nausea, vomiting, diarrhea, poor appetite, and fatigue, and higher levels could make these worse.
The important part: this is based only on lab (test-tube) work, not on real people, so it's theoretical. Still, since lonafarnib is a serious, carefully dosed medication, I'd play it safe. Please check with your doctor or pharmacist before adding any Ceylon Leadwort.

Here's the thing with Lonafarnib (Zokinvy): your body relies heavily on an enzyme called CYP3A4 to clear it out. Ceylon Leadwort contains plumbagin, which in lab studies appears to slow that enzyme down. If that effect carried over into people, it could let lonafarnib build up higher than intended. With this drug, that may mean more nausea, vomiting, diarrhea, appetite loss, fatigue, or changes in liver enzymes.
I want to be honest about the evidence though: this is based only on test-tube data, not real patients, so it's theoretical at this point. Still, lonafarnib already comes with strict instructions to avoid CYP3A4 blockers, so I'd play it safe. Please check with your pharmacist or doctor before pairing these two.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

What Is Lonafarnib Used For?
Brand name: Zokinvy
Zokinvy (lonafarnib) is approved for patients 12 months of age and older who have a body surface area of 0.39 m² or above. It is used to reduce the risk of mortality in Hutchinson-Gilford Progeria Syndrome (HGPS) — a rare, severe premature-aging disorder.
Zokinvy is also used to treat processing-deficient Progeroid Laminopathies — a related group of rare genetic conditions — specifically those caused by a heterozygous LMNA mutation with progerin-like protein buildup, or by homozygous or compound heterozygous ZMPSTE24 mutations. It is not indicated for other Progeroid Syndromes or processing-proficient Progeroid Laminopathies.
