Mavacamten falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the situation with Camzyos (mavacamten) and Ceylon Leadwort. Your body relies heavily on a liver enzyme called CYP3A4 to clear mavacamten out of your system. In lab studies, plumbagin (the main active compound in Ceylon Leadwort) appears to slow that enzyme down. If that holds true in people, mavacamten could build up to higher levels than intended.

Why that matters: mavacamten is a drug where the dose is carefully fine-tuned to gently weaken an overly strong heart muscle. Too much can drop your heart's pumping ability too far, leaving you tired, short of breath, dizzy, or with swelling. That's why your doctor checks echocardiograms regularly. Right now this is only theoretical, based on lab data, not real patient reports. Still, given how precisely mavacamten is dosed, please check with your cardiologist or pharmacist before combining them rather than guessing.

It blocks CYP3A4, the liver enzyme that clears mavacamten, and too much of this drug can weaken the heart's pumping strength, the very problem careful dosing exists to prevent.”
