Why this applies to nilotinib

nilotinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Your body relies on a liver enzyme called CYP3A4 to clear nilotinib out of your system. Plumbagin, the active compound in Ceylon Leadwort, has been shown to slow that same enzyme down, at least in lab dish studies. If it does the same thing in people, nilotinib could clear more slowly and build up to higher levels than expected.

Why that matters: nilotinib already carries a warning about heart rhythm changes (a prolonged QT interval), plus it can cause low blood counts, nausea, rash, and tiredness. Higher levels could make these more likely. Keep in mind this concern comes only from lab research, not real patients, so we honestly don't know how strong it is in real life. Since nilotinib is a serious cancer medicine, please run any Ceylon Leadwort use past your oncologist or pharmacist first.

Plumbagin, a constituent of Ceylon leadwort, has demonstrated immunosuppressant activity in animal research. Theoretically, Ceylon leadwort might interfere with immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and others.

Here's the honest picture with nilotinib (Tasigna) and Ceylon Leadwort. Nilotinib is a targeted leukemia drug, a tyrosine kinase inhibitor, and it isn't really an immune-suppressing drug in the same way the medicines on this interaction list are. So this caution is a loose fit at best, and it rests only on lab and animal work showing that plumbagin (the active compound in Ceylon Leadwort) can dampen immune activity. That makes this theoretical, not something proven in people taking nilotinib.

My bigger concern is simpler: Ceylon Leadwort hasn't been studied well in humans, and concentrated supplement doses of plumbagin can irritate tissues. Since nilotinib treats a serious condition with its own real side effects, please don't add this supplement without checking first with your oncology team or pharmacist.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. About Nilotinib

What Is Nilotinib Used For?

Brand name: Tasigna

  • Nilotinib is used to treat certain types of chronic myeloid leukemia (CML; a type of cancer of the white blood cells) who have recently found to have this condition in adults and children 1 year of age and older. It is also used to treat certain types of CML in adults whose disease could not be treated successfully with imatinib (Gleevec) or adults who cannot take imatinib because of side effects. Nilotinib is also used to treat certain types of CML in children 1 year of age or older whose disease could not be treated successfully with other tyrosine kinase inhibitor therapies or who cannot take these medications because of side effects. Nilotinib is in a class of medications called kinase inhibitors. It works by blocking the action of the abnormal protein that signals cancer cells to multiply. This helps to stop or slow the spread of cancer cells.
