Why this applies to Nirmatrelvir, Ritonavir

Nirmatrelvir, Ritonavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's an interesting wrinkle with Paxlovid. The ritonavir half of this combo is actually there on purpose to block CYP3A4, which slows the breakdown of nirmatrelvir and keeps its levels high enough to fight the virus. Lab studies suggest plumbagin (the main compound in Ceylon Leadwort) can also block CYP3A4. So in theory, layering it on top of Paxlovid might push nirmatrelvir levels a bit higher, which could mean more of the usual side effects like an altered or metallic taste, nausea, diarrhea, or muscle aches.

I want to be honest, though: this is only seen in test-tube research, never reported in real people, and Paxlovid is a short five-day course. Still, since it already plays heavily on this enzyme, run any Ceylon Leadwort use past your pharmacist or doctor first.
