Why this applies to Vorapaxar

Vorapaxar falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the situation with Vorapaxar (Zontivity) and Ceylon Leadwort. Your body relies on a liver enzyme called CYP3A4 to clear vorapaxar out of your system. The plumbagin in Ceylon Leadwort has been shown in lab dish studies only to slow that enzyme down. If that were to happen in real life, vorapaxar could build up to higher levels than intended.

Since vorapaxar is a blood thinner that stops platelets from clumping, more drug in your system could mean a higher chance of bleeding or bruising, which already lasts a long time with this medicine. That said, this has never been reported in actual people, so it's theoretical for now. Still, vorapaxar and bleeding risk are nothing to gamble with, so please check with your pharmacist or doctor before combining the two.

Vorapaxar also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

There is some concern that Ceylon leadwort might potentiate the effects of anticoagulant and antiplatelet drugs and possibly increase the risk of bleeding. Ceylon leadwort root extract has been shown to decrease platelet adhesion and prolong bleeding in animals. However, this effect has not yet been demonstrated in humans. Until more is known, use cautiously in patients taking anticoagulant or antiplatelet drugs. Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here's the thing to keep in mind: Vorapaxar (Zontivity) is already a potent antiplatelet medicine, and it's usually taken alongside aspirin and clopidogrel. That means you're working with a real blood-thinning effect to start with. Ceylon Leadwort has been shown in animal studies to make platelets less sticky and to lengthen bleeding time, so stacking it on top could, in theory, push you toward easier bruising, nosebleeds, bleeding gums, or harder-to-stop bleeding.

I want to be honest: this concern comes from lab and animal research, not from human studies, so the real-world risk isn't well proven. Still, because Vorapaxar already raises bleeding risk on its own, I'd play it safe. Please check with your pharmacist or doctor before adding this supplement.

Zontivity (vorapaxar) is used to lower the risk of heart attack, stroke, cardiovascular death, and the need for urgent procedures to open blocked arteries. It is prescribed for people who have already had a heart attack or who have peripheral arterial disease (PAD) — a condition where arteries outside the heart become narrowed or blocked.

Zontivity is not used alone. It is given alongside aspirin and/or clopidogrel as part of a broader antiplatelet treatment plan.
