Amitriptyline falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cinchona interacts with.

Theoretically, taking cinchona might inhibit cytochrome P450 2D6 (CYP2D6) and increase levels of drugs metabolized by this enzyme.
Quinidine, a constituent of cinchona, inhibits CYP2D6-mediated metabolism in humans.

Here's the concern with pairing cinchona bark and amitriptyline. Your liver relies on an enzyme called CYP2D6 to break down amitriptyline and clear it from your system. Cinchona contains quinidine, a compound that's known to block that very enzyme. If that enzyme is slowed down, amitriptyline can build up higher than expected in your body.

What might that feel like? More of the typical side effects, things like drowsiness, dry mouth, constipation, blurry vision, dizziness when you stand up, and a faster heartbeat. I'll be honest, this is a theoretical interaction based on how the chemistry works, not something proven in studies, so don't panic. Still, please check with your pharmacist or doctor before combining them rather than changing anything on your own.

Theoretically, taking cinchona with other QT interval-prolonging drugs might cause an additive effect and increase the risk of ventricular arrhythmias.
Quinidine and quinine, which are constituents of cinchona, prolong the QT interval.

Here's the thing to know about pairing cinchona bark with amitriptyline (Elavil): both can stretch out the heart's electrical recharge time, what we call the QT interval. Cinchona naturally contains quinine and quinidine, and amitriptyline already has its own tendency to affect heart rhythm. Put them together and that effect could add up, which in rare cases raises the risk of a dangerous irregular heartbeat.

I'll be honest with you, this is a theoretical concern based on how these compounds behave, not something proven in studies. But amitriptyline can also cause dizziness, a racing or pounding heart, and fainting, so I wouldn't combine them on your own. Please check with your pharmacist or doctor first, especially if you have any heart history.
