Ondansetron falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Fever Bark interacts with.

Theoretically, fever bark might increase the levels and clinical effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of fever bark, inhibits CYP2D6 enzyme activity. The effects of fever bark itself are unclear.

Here's the gist: your body uses a liver enzyme called CYP2D6 to help clear ondansetron. Fever bark contains a yohimbine-like compound that, in lab dish studies, appears to slow that enzyme down. If it does the same thing in people, ondansetron could linger a bit longer and its effects (and side effects) might be amplified. With ondansetron that could mean more headache, constipation, dizziness, or tiredness, and in rare cases changes to your heart rhythm (QT prolongation).

I want to be honest, though: this is only a theoretical concern based on test-tube data, and how fever bark itself behaves in real people isn't well understood. Fever bark also carries its own risks, so I'd skip it unless your doctor approves. Please check with your pharmacist or doctor before combining the two.

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections. There is very little reliable human research to support these uses, and the bark contains potent alkaloids that may cause serious side effects. It is best avoided unless used under direct medical supervision.

What Is Ondansetron Used For?

Brand name: Zofran

Ondansetron is used to prevent nausea and vomiting in several situations. Oral forms (tablets, orally disintegrating tablets, and oral solution) are approved to prevent nausea and vomiting from highly or moderately emetogenic (nausea-causing) cancer chemotherapy, from radiation therapy to the abdomen or whole body, and from surgery.

Ondansetron injection (given intravenously or intramuscularly in a clinical setting) is approved to prevent nausea and vomiting from emetogenic cancer chemotherapy — including high-dose cisplatin — in adults and children as young as 6 months old, as well as to prevent postoperative nausea and vomiting.
