CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Tadalafil
Tadalafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Melatonin interacts with.
Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: your body uses an enzyme called CYP3A4 to clear tadalafil (Cialis) out of your system. Some lab studies hint that certain batches of melatonin might slow that enzyme down a little. In theory, that could let tadalafil linger and build up slightly, which might nudge up its typical side effects like headache, flushing, a stuffy nose, back pain, or a bigger dip in blood pressure.
I want to be honest with you though: this is only based on test-tube research, not real patients, and it's rated as minor. For most people the effect is probably small or none at all. Still, if you ever feel dizzy, lightheaded, or get a pounding headache when combining them, mention it to your pharmacist or doctor before changing anything.

What Is Tadalafil Used For?
Brand names: Cialis, Tadliq
Tadalafil tablets (including Cialis and generic tadalafil) are used to treat erectile dysfunction (ED), the symptoms of benign prostatic hyperplasia (BPH — an enlarged prostate), or both conditions together. When used with the drug finasteride to start BPH treatment, this combination is recommended for up to 26 weeks.
Adcirca, Alyq (tablets), and Tadliq (oral suspension) are indicated specifically for pulmonary arterial hypertension (PAH — high blood pressure in the lungs), to improve the ability to exercise.
