Alogliptin, Metformin falls under ANTIDIABETES DRUGS — the group Milk Thistle interacts with.

Taking milk thistle with antidiabetes drugs may increase the risk of hypoglycemia.
Clinical research shows that milk thistle extract, alone or along with tree turmeric extract, can lower blood glucose levels and glycated hemoglobin (HbA1c) in patients with type 2 diabetes, including those already taking antidiabetes drugs. Additionally, animal research shows that milk thistle extract increases the metformin maximum plasma concentration and area under the curve and decreases the renal clearance of metformin, due to inhibition of the multi-drug and toxin extrusion protein 1 (MATE1) renal tubular transport protein.

Kazano combines two diabetes medicines, alogliptin and metformin, that work together to bring your blood sugar down. Adding milk thistle to the mix can pull in the same direction. Some clinical studies show silymarin (milk thistle's main ingredient) can lower blood sugar and HbA1c on its own, so stacking it on top of your medication may push your numbers a little too low.

There's also a metformin-specific piece. Lab and animal research suggests milk thistle can slow how your kidneys clear metformin (by blocking a transporter called MATE1), letting more of it build up. That could mean more shaky, sweaty, dizzy low-sugar episodes, plus stomach upset. The evidence isn't airtight, so don't panic, but check with your pharmacist or doctor before combining them.
