Why this applies to Azilsartan

Azilsartan falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the gist for Edarbi (azilsartan). Your body partly clears this blood pressure medicine using a liver enzyme called CYP2C9. In a test tube, milk thistle might slow that enzyme down a little, which in theory could let azilsartan build up slightly. If that happened, you might notice more of its usual effects, like a bigger drop in blood pressure, lightheadedness, or dizziness when you stand up.

That said, the evidence here is shaky and conflicting. Some human studies show milk thistle does not meaningfully block CYP2C9, so most people won't run into a real problem. I wouldn't lose sleep over it, but it's still worth mentioning to your pharmacist or doctor, especially if you check your blood pressure and see it running low.

What Is Azilsartan Used For?

Brand name: Edarbi

Azilsartan medoxomil tablets — sold as both the generic Azilsartan Medoxomil and the brand Edarbi — are approved to treat high blood pressure (hypertension) in adults. Bringing blood pressure down lowers the risk of serious cardiovascular events, especially stroke and heart attack.

The medication can be used on its own or alongside other blood pressure medicines. It is not a cure for high blood pressure, and managing the condition fully often involves lifestyle steps like limiting sodium, exercising, and quitting smoking.
