Azilsartan, Chlorthalidone falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the short version on pairing milk thistle with Edarbyclor (which combines azilsartan, a blood pressure medicine, with chlorthalidone, a water pill). Azilsartan is partly handled by a liver enzyme called CYP2C9, and there's a theory that milk thistle might slow that enzyme down. If that happened, a little more azilsartan could hang around, which in theory might nudge your blood pressure lower and add to side effects like dizziness, lightheadedness, or feeling drained.

The good news: the evidence here is shaky and conflicting, and a real-world reaction is considered unlikely. Most people won't notice anything. Still, since this combo already lowers blood pressure, it's worth a quick chat with your pharmacist or doctor before adding milk thistle, and keep an eye on how you feel.
