Theoretically, milk thistle might affect the clearance of drugs that undergo glucuronidation.
Laboratory research shows that milk thistle constituents inhibit uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Other laboratory research suggests that a milk thistle extract of silymarin might inhibit beta-glucuronidase, although the significance of this effect is unclear.

Here's the gist with Duavee (conjugated estrogens plus bazedoxifene). Both of these hormones are cleared from your body partly through a process called glucuronidation, where the liver attaches a sugar tag so the drug can be flushed out. Lab studies hint that milk thistle's silymarin compounds may slow that tagging enzyme down a bit. If that happened in real life, your hormone levels could drift a little higher, which in theory might mean more of the usual estrogen-type effects like breast tenderness, bloating, nausea, or spotting.

I want to be honest though: this is based only on test-tube and animal research, not real patients, so the actual risk is uncertain and probably small. Still, since hormone therapy carries its own cautions, it's worth a quick chat with your pharmacist or doctor before pairing them.

Here's the gist with Duavee (which combines conjugated estrogens with bazedoxifene to ease hot flashes and protect bones). The concern with milk thistle is purely theoretical and comes from lab and animal work, not from people. In those studies, a compound in milk thistle called silymarin attached to estrogen receptors. The idea is that it could compete with estrogen at those receptors and slightly blunt how well your therapy works.

In real life, this is rated as a minor interaction, so I wouldn't lose sleep over it. At most, you might wonder if your hot flashes feel less controlled. We just don't have human evidence showing this actually happens. If you want to use milk thistle, please run it past your pharmacist or doctor first, since the supplement isn't doing much for the liver in most cases anyway.

P-GLYCOPROTEIN SUBSTRATES

Bazedoxifene Acetate, Conjugated Estrogens also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, milk thistle might increase the absorption of P-glycoprotein substrates. However, this effect does not seem to be clinically significant.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.

Here's the gist for Duavee (the combo of conjugated estrogens and bazedoxifene). One of the proteins that helps move this medication around your body is called P-glycoprotein, and milk thistle might slow that protein down a bit in lab tests. In theory, that could let slightly more of the drug get absorbed, which in turn could nudge up estrogen-type effects like breast tenderness, bloating, nausea, or spotting.

That said, this is very much a theoretical concern. The real-world evidence is weak, and a study using a similar P-glycoprotein medication (digoxin) showed no meaningful change. So for most women, taking milk thistle with Duavee is unlikely to cause a problem. Still, it's worth a quick mention to your pharmacist or doctor, just so they have the full picture.

While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the short version on pairing milk thistle with Duavee (conjugated estrogens plus bazedoxifene). Part of this medicine is handled by a liver enzyme called CYP3A4, and milk thistle has been looked at as something that might slow that enzyme down. In theory, if it did, the estrogen portion could linger a bit longer and you might notice more estrogen-type effects like breast tenderness, bloating, nausea, or spotting.

But here's the reassuring part: the actual human studies are conflicting, and good pharmacokinetic research suggests milk thistle usually does not meaningfully change CYP3A4 medicines. What Is Estrogen and Bazedoxifene Used For?

Brand name: Duavee

  • Estrogen and bazedoxifene tablets are used to treat hot flashes (sudden feelings of warmth, especially in the face, neck, and chest) in women who are experiencing menopause (stage of life when menstrual periods become less frequent and stop and women may experience other symptoms and body changes). Estrogen and bazedoxifene tablets are also used to prevent osteoporosis (condition in which the bones become thin and weak and break easily) in women who have undergone menopause. Estrogen is in a class of medications called hormones and bazedoxifene is in a class of medications called estrogen agonist–antagonists. Estrogen works by replacing estrogen that is normally produced by the body. Bazedoxifene is used to block the action of estrogen on the lining of the uterus, decreasing the risk of overgrowth that may lead to cancer.
