Clomipramine falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the gist for Clomipramine (Anafranil): your body uses several liver enzymes to clear it, and one of them is CYP2C9. The thinking is that milk thistle might slow that enzyme a little. If it did, clomipramine could clear more slowly and build up, which can mean more of its usual side effects like drowsiness, dry mouth, constipation, blurry vision, or a faster heartbeat.
The honest part: the evidence here is shaky and conflicting. Some lab studies hint at a block, but better human studies show milk thistle doesn't meaningfully affect CYP2C9. This is rated a minor and unlikely interaction. Still, if you take clomipramine and want to start milk thistle, run it by your pharmacist or doctor first, and mention any new side effects.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the short version: this is a very minor, mostly theoretical concern. Clomipramine is partly processed by a liver enzyme called CYP3A4, and milk thistle might, in theory, slow that enzyme down. If it did, a bit more clomipramine could linger in your body, which could nudge up its usual side effects (dry mouth, constipation, drowsiness, blurred vision, or a faster heartbeat).
But the actual human studies are reassuring. In careful testing, milk thistle didn't meaningfully change how the body handles other CYP3A4 drugs, and the warning signals came mostly from strong cancer medicines, not antidepressants. Clomipramine is not a prodrug, so it doesn't flip the usual direction. Still, it's wise to mention milk thistle to your pharmacist or doctor so they can keep an eye on how you feel.
