Moxifloxacin falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Milk Thistle interacts with.

Milk thistle may inhibit one form of OATP, OATP-B1, which could reduce the bioavailability and clinical effects of OATP-B1 substrates.
In vitro research shows that milk thistle inhibits OATP-B1. Two case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are OATP substrates, including sorafenib and methotrexate. OATPs are expressed in the small intestine and liver and are responsible for the uptake of drugs and other compounds into the body. Inhibition of OATP may reduce the bioavailability of oral drugs that are substrates of OATP.

Here's the basic idea: milk thistle (its silymarin compounds) may block a gut and liver transporter called OATP-B1, which helps shuttle certain drugs into your body. If moxifloxacin relies on that transporter, milk thistle could slightly reduce how much of the antibiotic gets absorbed. In theory, less drug in your system might mean a weaker punch against the infection you're trying to clear.

I want to be honest about the strength of this one. It rests on lab and animal data, not solid human studies, and it's rated minor. For most people taking a short antibiotic course, this likely won't be a real-world problem. Still, since you don't want anything undercutting an infection treatment, mention the milk thistle to your pharmacist or doctor before combining them.

About Moxifloxacin

What Is Moxifloxacin Used For?

Brand name: Avelox

Moxifloxacin tablets and intravenous (IV) injection are approved for adults 18 and older to treat bacterial infections including community-acquired pneumonia, uncomplicated and complicated skin infections, complicated abdominal infections, plague, acute bacterial sinusitis, and acute bacterial flare-ups of chronic bronchitis. Note: moxifloxacin should only be used for sinusitis and chronic bronchitis flare-ups when no other antibiotic option is available, due to the risk of serious side effects.

Moxifloxacin ophthalmic solution (eye drops) — including the brand Vigamox — is a separate, topical formulation used only in the eye to treat bacterial conjunctivitis (pink eye) caused by susceptible bacteria. The eye drop form is not used for body-wide infections.
