Why this applies to Zileuton

Zileuton falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the gist for Zileuton (Zyflo): your body partly clears it using a liver enzyme called CYP2C9, and there's a theory that milk thistle (silymarin) might slow that enzyme down. If that happened, more zileuton could linger in your system, which in theory could nudge up side effects like headache, nausea, stomach upset, or changes in liver enzyme tests.

The honest part: this one is shaky. Some lab studies hint at it, but better human research suggests milk thistle doesn't meaningfully block CYP2C9. The interaction is rated minor and unlikely. Since zileuton already needs liver monitoring, I'd just mention to your pharmacist or doctor that you take milk thistle, especially if you use a concentrated daily supplement rather than an occasional dose.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist for these two: your body uses an enzyme called CYP3A4 to help clear zileuton, and milk thistle has been looked at as something that might slow that enzyme down. If it did, zileuton could build up a little, which in theory might mean more of its usual side effects, things like nausea, stomach upset, or headache.

The honest truth, though, is that the evidence here is thin. The lab studies disagree with each other, and careful human studies showed milk thistle didn't meaningfully change how the body handles similar CYP3A4 drugs. So this is rated minor and unlikely. Zileuton is not a prodrug, so we're not worried about it suddenly stopping working. Still, since zileuton already needs liver monitoring, it's worth a quick word with your pharmacist or doctor before adding milk thistle.

Milk thistle is a popular herbal supplement most often used for liver health, and its main active component is a group of compounds called silymarin. While it is generally well tolerated, the scientific evidence that it meaningfully improves liver disease or other conditions is mixed and far from conclusive. About Zileuton

What Is Zileuton Used For?

Brand names: Zyflo, Zyflo CR

Zileuton is used for the ongoing prevention and treatment of asthma in adults and adolescents aged 12 and older. It is a maintenance medicine — meaning it is taken regularly to keep asthma under control, not to stop an asthma attack once it has started.

Zyflo (the tablet form) and zileuton extended-release tablets are both approved for this same purpose. Neither form should be used to treat a sudden asthma attack. However, both can be continued during a flare-up of asthma while you also use your rescue inhaler.
